Default Domino effects

Had one of those idiot incidents the other day (me being the idiot). Was just exiting an intersection
following a fairly tight stream of cars in the left lane, noticing that the right lane had opened up I
decided to change lanes. Everyone was moving at a fairly steady pace, so do a head check to make
sure I'm clear on the right, look back, and whoa, the van in front of me is completely stopped and
I'm still rolling at about 20mph with about 8 feet between me and the van. At this point, I
instinctively swerved to the right just barely missing the rear end of the van (thinking oh %%%% I'm
going to lose a mirror on this one).

I don't know many countless times I've been in this situation before, where you have a line of cars all
following each other too close, which almost always results in someone slamming on their breaks.
I imagine I was thinking I still had time to change lanes before this occured. Definitely need to
keep that safe distance between you and cage in front of you, even at slower speeds. The thought
of rear ending a car even at slow speeds seems like it might be real painful.
