Default Mirrors focused on where?

I'm a little confused on the correct position for my mirrors as far as what views should I have while riding. My manual says I should see a small part of my shoulder. The problem is if I do that I can't see the cars to the left or right of me until it's too late. When I place the mirrors where I can see the left and right of me, I can't see the vehicle directly behind me and thats the number one threat they tell me to watch for. If I lean a little to one side I can see behind me but I don't know if thats safe to do while riding. Im sure its a simple fix just want to be on the right track and see what the norm is for riders.
