Default 767 Engine came apart at full power

Just thought I would pass this along...

At 1230 PM, Friday, June 2, 2006 at LAX. American Airlines Boeing 767
doing a high power engine run had a #1 engine HPT failure.
HPT let go and punctured left wing, #2 engine, peppered fuselage and set
fire to the aircraft. The turbine disk exits the engine and
slices through the aircraft belly and lodges in the outboard side of the
#2 engine.

Hmm looks like TO5 failure ...We never had our GE Lm2500 come apart like that.
What model and who made it?

Was this an inherent design failure or did the mechanics fail to follow Technical Orders?

I still prefer Boeing airframes over Airbus - hate the way airbus sounds when its "adjusting" trim and preflight. Always seems like there is some kind of servo motor twitching and cranking down.

People...these are why we have regualtions and technical orders in place.
The technicians that were performing this work must check these engines at full power. The jet engines (S2 can chime in) are tested under full power for reliabilty. Any type of malfunction or repair requires a full power test. The theory is to have it fly apart chained to the ground than in the air.....so those that are afraid of flying..how many commercial planes have been put into the drink or the side of a mountain lately....please tell me.

I can assure that the vehicle fatality rate over the last decade over laps any commercial air fatality. hell lets make that the last three decades. Lets even throw in world wide.....not counting man inflicted problems.

You are right on Pete. I am not sure of the particulars concerning this failure but this is the reason this type of testing is preformed. The FAA imposes strict test guidelines at certain intervals to ensure this sort of thing does not happen at the wrong time, rest assured. This incident will be investigated and the findings will most likely result in refinements to the maintenance programs. Happens all the time, typically not so dramatic and with as much force.

I am not an engine guy; the only reason to have engines, wings and fuselages is to keep the Avionics airborne.

The newer Airbus are fly-by-wire so there is a servo motor moving the flight controls, as opposed to a cable system.

I'm not an engine guy either, which is why I posted the question. I'm an airfield guy. AFAIK, even after extensive testing, design weaknesses can come up after production. As living documents, regs and T.O.'s should be periodically updated to address weaknesses and improve processes. I'm not much for speculation, but I'll agree with Pete as the most likely cause would be the human factor. I also wonder if foreign object damage (FOD) may play a role as well. For instance, a tool inadvertently left in the engine nacelle. Not sure about the civilian side, but AF mechanics are expected to account for all of their tools prior to the end of their shift.
