It has happened to me 2 or 3 times ( the neutral to first problem ). I have not seen it since I started adjusting my clutch regularly.

The from second to third problem sounds like the shifter needs to be adjusted to me as well.
Oh, okay. That sounds like a simple solution. Hopefully I'll actually remember it should it happen again
