wouldn't a call or walk-in visit to your local DMV get you all the answers and forms you'd need? i'd assume you'd need a few pieces of your own ID. i was at my local DMV a few weeks ago and a lady had lost her ownership as well - wasn't a big deal for her to get a new one, they just needed a few pieces of ID, looked up her record on the computer, and probably charged her a few bucks for it.
i would assume that a passport or other gov't issued ID with your pic and signature would be the minimum, probably two pieces. possibly also a document with your name and mailing address, like a credit card statement or phone bill that gets mailed to your residence.
